El desarrollo de acondicionamientos con menor toxicidad, el mejor control y conocimiento de la enfermedad injerto contra hu\u00e9sped (eich) y del efecto injerto contra tumor (eict), junto al desarrollo de herramientas efectivas para monitorizar el trasplante, est\u00e1n permitiendo mejorar los resultados del alo-tph, ampliando el n\u00famero de indicaciones y de pacientes que pueden beneficiarse del mismo. Sin embargo, a pesar de los avances, cerca del 20-30% de los pacientes que reciben un alo-tph fallecen de complicaciones derivadas del procedimiento, principalmente eich.  el objetivo de la presente tesis doctoral es el estudio de polimorfismos gen\u00e9ticos en la evoluci\u00f3n de pacientes sometidos a alo-tph emparentado hla id\u00e9ntico. para ello analizamos tres sistemas polim\u00f3rficos: hla, microsat\u00e9lites y dos mol\u00e9culas de adhesi\u00f3n.  respecto al sistema hla, describimos las frecuencias al\u00e9licas y haplot\u00edpicas de nuestra poblaci\u00f3n, aportando un grupo control de nuestra regi\u00f3n. Asimismo, observamos dos haplotipos frecuentes descritos previamente con frecuencias <1% en espa\u00f1a. por \u00faltimo, analizamos la influencia del sistema hla en la predisposici\u00f3n y pron\u00f3stico en pacientes con mieloma m\u00faltiple, observando una mayor incidencia del alelo drb1*01 con formas m\u00e1s indolentes de la enfermedad.  respecto al an\u00e1lisis con microsat\u00e9lites, describimos las frecuencias al\u00e9licas de 15 str de nuestra regi\u00f3n, localizando los cuatro marcadores de mayor utilidad para el an\u00e1lisis del quimerismo. analizando el quimerismo secuencial en alo-tph con intensidad reducida, describimos un grupo de pacientes con alto riesgo de reca\u00edda cuyo pron\u00f3stico puede mejorar mediante inmunoterapia adicional. finalmente, una mayor discrepancia gen\u00e9tica entre donante y receptor se correlaciona con mayor severidad de eicha y peor pron\u00f3stico.  para concluir, la presencia de polimorfismos en la mol\u00e9cula de adhesi\u00f3n icam-1 parece no influir en la evoluci\u00f3n del alo-tph, mientras que aquellos en pecam-1 podr\u00edan influir en la severidad de eicha.\n\n\n\n&nbsp;\n\n\n<h3>Datos
