Se estudio la densidad mineral osea mediante absorciometria dual por rx (dxa) en 75 pacientes cirroticos clasificados por etiolog\u00eda y gravedad de la enfermedad (43 alcoholicos: 13 child b, 16 child b, 14 child c; y 32 viricos: 16 child a, 10 child b y 6 child c). Ademas, como parametros bioquimicos relacionados con el metabolismo mineral, se determinaron los niveles sericos de calcio, magnesio, fosforo, pth mm, pth int, vitamina d, osteocalcina, fosfatasa alcalina osea, factor de crecimiento relacionado con la insulina (igf-1), y testosterona. En orina se determino el calcio, fosforo, magnesio, deoxipiridinolina libre y creatinina. Como controles para los parametros bioquimicos se utilizaron 24 varones sanos de similar edad media. En nuestros resultados se encontro una alta preValencia de osteoporosis en los enfermos cirroticos (46%), independientemente de la etiolog\u00eda alcoholica o virica de la enfermedad. La perdida de masa osea fue mayor en el hueso trabecular (columna lumbar).  no se encontraron diferencias en la osteopenia entre los enfermos cirroticos de etiolog\u00eda alcoholica y virica.  podemos concluir que la osteopenia y la osteoporosis de la cirrosis hepatica de origen alcoholico o virico parece ser de origen multifactorial. Asi, segun nuestro estudio influyen la gravedad de la hepatopatia, el grado de hipertension portal, la disminucion del igf-1 serico, el descenso en los niveles de testosterona libre, la hipovitaminosis d y el hipoparatiroidismo. En la cirrosis alcoholica la ingesta etilica influye en la osteopenia.  el remodelado oseo de la cirrosis hepatica parece ser de alto turnover (alto recambio), en base a los niveles aumentados de fosfatasa alcalina osea serica y deoxipiridinolina libre en orina.
