We conducted an open single arm phase i trial infusing bmncs into the posterior spinal cord funiculus. Safety was defined as the absence of serious transplant-related adverse events. Eleven patients were included. There were no severe transplant-related adverse events. No acceleration in the rate of decline of forced vital capacity, als-frs, norris, or mrc scales was observed. Four patients died on days for reasons unrelated to the procedure. A greater number of motoneurons\/section in the treated compared with the untreated segments (4.2\u00c2\u00b10.8 and 0.9\u00c2\u00b10.3 respectively) was observed. Those motoneurons were surrounded by cd90+ cells and did not show degenerative deposits. This study confirms the safety of intraspinal infusion of bmnc in als and provides evidence of their neurotrophic activity.  nuestro objetivo fue comprobar la seguridad de la infusi\u00f3n intraespinal de c\u00e9lulas mononucleadas de m\u00e9dula \u00f3sea aut\u00f3loga (bmncs) y buscar signos de neurotrofismo celular en pacientes con esclerosis lateral amiotr\u00f3fica (ela). Realizamos un ensayo fase i, infundiendo bmncs en el cord\u00f3n posterior medular. La seguridad se defini\u00f3 como la ausencia de eventos adversos severos relacionados con el tratamiento. Incluimos once pacientes. No hubo eventos adversos severos relacionados con el tratamiento. Tampoco hubo aceleraci\u00f3n del deterioro de la capacidad vital forzada ni de las escalas als-frs, norris o mrc. Cuatro pacientes fallecieron por razones no relacionadas con el tratamiento. Se observ\u00f3 un mayor n\u00famero de motoneuronas\/secci\u00f3n en los segmentos tratados que en los no tratados (4.2\u00c2\u00b10.8 y 0.9\u00c2\u00b10.3 respectivamente). Estas motoneuronas estaban rodeadas por c\u00e9lulas cd90+ y no presentaban dep\u00f3sitos degenerativos.
