The availability of repetitive stimuli opens the possibility of a muscle training modality with little cardio respiratory strain, specially suited for severe copd patients with more accused muscle dysfunction and greater difficulties in performing pulmonary rehabilitation.Efficiency of this treatment was studied in a randomised controlled clinical trial in which 15 minutes bilateral repetitive magnetic neuromuscular stimulation treatment sessions were programmed three times a week during eight weeks. The 10 treated severe copdpatients showed a 17.5% increase in quadriceps muscle strength respect to baseline as well as a 23m increase in six minutes walking distance. In comparison with the 8 control patients, the treated group achieved greater improvements in quality of life questionnaire scores. Muscle biopsies showed that training did not modify redox balance, whilst inducing a significant increase in type i fibre sizes.Our results confirm that repetitive magnetic neuromuscular stimulation is a feasible training technique, with benefits in severe copd patients, specially in those circumstances where traditional pulmonary rehabilitation cannot be performed.Esta tesis doctoral recoge la investigaci\u00f3n realizada con el fin de determinar si la aplicaci\u00f3n de un est\u00edmulo neuromuscular repetitivo, aplicado por un equipo medtronic magpro, es capaz de producir un efecto de entrenamiento similar a las modalidades cl\u00e1sicas de rehabilitaci\u00f3n respiratoria por el ejercicio para los pacientes con epoc.Con este fin se estudi\u00f3 el rendimiento del equipo citado, comparado con el electroim\u00e1n magstim est\u00e1ndar que se utiliza para la medida del twitch supram\u00e1ximo del m\u00fasculo cu\u00e1driceps femoral. La disponibilidad de est\u00edmulos repetitivos abre la posibilidad de realizar un entrenamiento muscular con escasa exigencia cardiorrespiratoria adecuada para pacientes epoc graves, que presentan mayor disfunci\u00f3n muscular y potenciales dificultades para la rehabilitaci\u00f3n.La eficacia de este tratamiento fue estudiada mediante un estudio randomizado controlado en el que se aplic\u00f3 un protocolo de estimulaci\u00f3n magn\u00e9tica repetitiva durante 30 minutos, tres d\u00edas por semana, durante ocho semanas. Como hallazgos principales destacamos el aumento de la fuerza del cu\u00e1driceps, de un 17,5% respecto al valor basal en los 10 pacientes epoc severos tratados y un incremento de 23 m en la prueba de seis minutos de marcha. Estos pacientes presentaron cambios mayores en sus cuestionarios de calidad de vida respecto a los 8 controles. Comparadas las muestras de biopsia muscular nos observaron cambios en el equilibrio redox con la estimulaci\u00f3n magn\u00e9tica, aunque s\u00ed un incremento en el tama\u00f1o de las fibras de tipo i.Nuestros resultados corroboran que la estimulaci\u00f3n magn\u00e9tica repetitiva es una t\u00e9cnica de entrenamiento muscular factible, con efecto beneficioso en pacientes epoc graves,sobre todo en circunstancias en las que no es factible la rehabilitaci\u00f3n respiratoria tradicional.Doktore-tesi honek medtronic magpro ekipoak emandako estimulazio neuromuskular magnetiko errepikariak errehabilitazio klasikoak bezalako entrenamendu ondorioak eragiten dituen ikertzeko egin genituen ikerketa lanak biltzen ditu. Astean hiru aldiz, 30 minutuzko estimulazioa hartzen zuten 10 gaixoek oinarri mailaz gaineko %17,5 koadrizepseko indar gorakada erakusten zuten eta baita ere 23 m-ko hobekuntza 6 minutuko ibilera frogan, 8 kontrol-gaixoekin alderatuta. Tratatutako gaixoek ere izan zituzten aldaketa nabarmenagoak bizi kalitateko galdeketen puntaketetan.Gihar biopsiak konparatuta, esan daiteke estimulazio tratamenduak redox-desorekarik ez duela
