La principal funci\u00f3n del surfactante es la de reducir la tensi\u00f3n superficial y evitar el colapso pulmonar al final de la fase espiratoria.En humanos esta sustancia comienza a sintetizarse en el \u00faltimo tercio de la gestaci\u00f3n, por lo que muchos de los beb\u00e9s nacidos de forma prematura sufren el s\u00edndrome de dificultad respiratoria, que se caracteriza por la aparici\u00f3n de hipoxia, hipercarbia, acidosis, inflamaci\u00f3n pulmonar, enfermedad pulmonar cr\u00f3nica, secuelas neurol\u00f3gicas y, en el peor de los casos, la muerte.El tratamiento actual consiste en introducir surfactante de origen animal (porcino, en la mayor\u00eda de los casos), inyect\u00e1ndolo a trav\u00e9s de un cat\u00e9ter por el tubo endotraqueal. Esta terapia, aunque muy efectiva, tiene asociados varios efectos adversos como, por ejemplo, la intubaci\u00f3n endotraqueal, la obstrucci\u00f3n de las v\u00edas a\u00e9reas, desaturaciones, hipercarbia, inestabilidad hemodin\u00e1mica, y fluctuaciones del flujo sangu\u00edneo cerebral; \u00e9stas \u00faltimas deben tenerse muy en cuenta, debido a que las fluctuaciones del flujo sangu\u00edneo cerebral han sido asociadas a da\u00f1o neurol\u00f3gico permanente.La terapia con aerosoles de surfactante podr\u00eda, en gran parte, paliar los efectos adversos de la forma de administraci\u00f3n actual. No obstante, el uso de aerosoles terap\u00e9uticos en neonatos es ineficiente, y el surfactante pulmonar es una sustancia muy viscosa y dif\u00edcil de nebulizar.En este trabajo, se ha testado el rendimiento de un novedoso dispositivo para la generaci\u00f3n de aerosoles intratraqueales de surfactante (in vitro), mostrando un rendimiento \u00f3ptimo del dispositivo. En una segunda fase (in vivo), se han llevado a cabo experimentos en corderos prematuros, con el fin de comparar la eficacia del surfactante en forma de aerosol con su m\u00e9todo actual de administraci\u00f3n cl\u00ednica (instilaci\u00f3n endotraqueal).Los resultados muestran que a nivel pulmonar la administraci\u00f3n de surfactante en forma de aerosol tiene una efectividad equivalente a su instilaci\u00f3n intratraqueal. No obstante, la administraci\u00f3n de surfactante en forma de aerosol parece mostrar ventajas en t\u00e9rminos de hemodin\u00e1mica cerebral, ya que al producirse los cambios a nivel pulmonar de una forma m\u00e1s gradual, los bruscos cambios hemod\u00ednamicos observados tras la instilaci\u00f3n intratraqueal pueden ser evitados.<\/p>\n<p>&nbsp;<\/p>\n<h3>Datos acad\u00e9micos de la tesis
