Aparte de provocar un enorme impacto social y abundantes gastos sanitarios, puede comprometer la viabilidad de la extremidad inferior afectada. En numerosas ocasiones, estas infecciones no son identificadas adecuadamente ni de forma precoz. La mayor\u00eda de las infecciones son causadas por bacterias gram positivas; las gram negativas se han asociado con casos de infecciones profundas, graves y polimicrobianas, y por tanto, de peor pron\u00f3stico. Objetivos: analizar la microbiolog\u00eda de las infecciones en \u00falceras de pie diab\u00e9tico en base a las caracter\u00edsticas cl\u00ednicas y generales de la muestra de estudio; comparando las infecciones producidas por bacterias gram positivas con las producidas por bacterias gram negativas. Dise\u00f1o: estudio observacional descriptivo, transversal y retrospectivo, con una muestra de 201 pacientes diab\u00e9ticos ulcerados y con sospecha cl\u00ednica de infecci\u00f3n (presencia de exudado purulento o de dos o m\u00e1s de los siguientes s\u00edntomas: dolor, rubor, calor, tumor, edema, linfangitis, olor, o presencia de gas en los tejidos) que acudieron a la unidad de pie diab\u00e9tico durante el per\u00edodo comprendido entre el mes de enero de 2009 y el mes de diciembre de 2010. Resultados: observamos diferencias estad\u00edsticamente significativas seg\u00fan el tipo de bacteria aislada en el cultivo microbiol\u00f3gico en los niveles de glucemia y hemoglobina glicosilada, en los antecedentes de retinopat\u00eda diab\u00e9tica y en la profundidad de la \u00falcera determinada por las escalas de wagner y texas. Conclusi\u00f3n: independientemente de la bacteria causante de la infecci\u00f3n, la identificaci\u00f3n precoz y la aplicaci\u00f3n de un tratamiento estandarizado consistente en antibioterapia guiada por cultivo, desbridamiento y curas locales homogeneiz\u00f3 el pron\u00f3stico, evitando diferencias entre ambos tipos de bacterias palabras clave: infecci\u00f3n, pie diab\u00e9tico, \u00falcera, bacteria gram negativa.\u00c2\u00bfgram-negative bacteria and their influence on the scarring of diabetic foot ulcers\u00c2\u00bf infection is one of the most common complications in diabetic foot syndrome. It causes a huge social impact and abundant health costs, and may compromise the viability of the affected lower extremity. In many cases, these infections are not adequately identified at an early stage. Most of the infections are caused by gram-positive bacteria; gram negative cases have been associated with deep, severe and polymicrobial infections, and therefore, with a worse prognosis. Objectives: to analyze the microbiology of infections in diabetic foot ulcers based on clinical and general characteristics of the sample, comparing infections caused by gram-positive bacteria with those produced by gram-negative bacteria. Design: observational, descriptive, cross-sectional and retrospective study with 201 ulcerated diabetic patients with clinical suspicious of infection (presence of purulent exudate or two or more of the following symptoms: pain, redness, heat, tumor, edema, lymphangitis, smell or presence of gas in the tissues) attending the diabetic foot unit during january 2009 to december 2010. Results: we observed statistically differences according the type of bacteria isolated in microbiological culture in glucose blood levels and glycated haemoglobin; in the backgrounds of diabetic retinopathy and in depth of the ulcer determined by wagner and texas scales. Conclussion: regardless of the bacteria causing the infection, early identification and implementation of a standarized culture-guided antibiotic treatment, debridement and local wound care
