La tos asistida manual (tam) y mec\u00e1nica &#8482; persigue preservar las funciones del aparato respiratorio. Objetivo:. Evaluar la eficacia del tratamiento con vibraciones transtor\u00e1cicas m\u00e1s insuflaci\u00f3n-exuflaci\u00f3n mec\u00e1nica frente a las t\u00e9cnicas manuales, para inducir tos asistida en pacientes pedi\u00e1tricos con pci grave o profunda. M\u00e9todo: 40 ni\u00f1os con pci hospitalizados por infecciones respiratorias con edad media 3,7 a\u00f1os (rango 1-13) fueron asignados aleatoriamente a dos grupos de tratamiento: tm y tam hasta 10 d\u00edas o remisi\u00f3n. Resultados: la tm fue m\u00e1s eficaz en  saturaci\u00f3n de ox\u00edgeno (p=0,000), confort ventilatorio (p=0,001), n\u00famero de sesiones (p=0,01) y reducci\u00f3n de s\u00edntomas (p=0,023). No existen diferencias en la frecuencia card\u00edaca y respiratoria. Conclusiones: la tm es m\u00e1s eficaz que la tam para reducir los s\u00edntomas, mejorar el confort ventilatorio y normalizar la saturaci\u00f3n oxihemoglob\u00ednica, logrando tales resultados en menos tiempo.  abstract: reoccurring respiratory infections are common in cerebral palsy (cp). Manually assisted cough (mac) and mechanical assisted cough (mc) aim to preserve the functions of the respiratory system. Objective: to evaluate the efficacy of transthoracic vibrations more mechanical insufflation-exsufflation compared to manual techniques to induce assisted cough in pediatric patients with severe or profound pci. Method: 40 children with cp hospitalized with respiratory infections, the average age is 3,7 years (ranging from 1-13) were randomly assigned to two treatment groups: mc and mac up to 10 days or remission. Results: mc was more effective for oxygen saturation (p=0.000), comfort in ventilation (p=0.001), number of sessions (p=0.01) and reduction in symptoms (p=0.023). No differences were detected in the respiratory and cardiac frequency.
