Es posible detectar y por tanto estudiar no invasivamente, el flujo de descendente anterior mediante ecocardiograf\u00eda transtor\u00e1cica y sondas de alta frecuencia, en un elevado porcentaje.  2. En pacientes con cardiopat\u00eda isqu\u00e9mica el estudio de la velocidad de flujo basal no es \u00fatil para discriminar la severidad de la estenosis, pero s\u00ed discrimina la calidad del vaso distal. Cuando el di\u00e1metro del vaso distal es inferior a 2 mm se observa disminuci\u00f3n de dicho flujo basal. El puente con mamaria produce un aumento de la velocidad de flujo sobre la situaci\u00f3n basal independientemente de la calidad del vaso distal (mayor o menor a 2 mm). Pero el puente con ami no se sigue de un aumento de la velocidad de flujo si la estenosis de la arteria nativa no es severa (inferior al 90%).  3. La reserva coronaria tras mamaria en pacientes sin datos de disfunci\u00f3n del injerto es inferior que la de los sujetos control e incluso de la de los sujetos con frcv. El factor que m\u00e1s influye en este bajo resultado en la rc es la severidad de la lesi\u00f3n nativa, de manera que cuando se pone una ami a una lesi\u00f3n inferior al 90% la rc obtenida es muy inferior a la obtenida si se revasculariza un da con lesi\u00f3n m\u00e1s severa. Adem\u00e1s, si la calidad del vaso distal es mala (di\u00e1metro inferior a 2mm) la rc es menor.  4. La ecocerdiograf\u00eda transtor\u00e1cica permite una valoraci\u00f3n funcional no invasiva de la cirug\u00eda de revascularizaci\u00f3n de la da con ami. De esta manera, se aprecian mejores resultados (m\u00e1s incremento de flujo y mayor rc) cuando se revasculariza una da con lesiones muy severas y buen vaso distal.
