{"id":31119,"date":"1997-01-01T00:00:00","date_gmt":"1997-01-01T00:00:00","guid":{"rendered":"https:\/\/www.deberes.net\/tesis\/sin-categoria\/estudio-densitometrico-de-la-remodelacion-osea-tras-protesis-no-cementada-de-cadera\/"},"modified":"1997-01-01T00:00:00","modified_gmt":"1997-01-01T00:00:00","slug":"estudio-densitometrico-de-la-remodelacion-osea-tras-protesis-no-cementada-de-cadera","status":"publish","type":"post","link":"https:\/\/www.deberes.net\/tesis\/ciencias-medicas\/estudio-densitometrico-de-la-remodelacion-osea-tras-protesis-no-cementada-de-cadera\/","title":{"rendered":"Estudio densitometrico de la remodelacion osea tras protesis no cementada de cadera."},"content":{"rendered":"<h2>Tesis doctoral de <strong> Juan  Jose Panisello Sebastia <\/strong><\/h2>\n<p>La remodelacion osea periprotesica tras la implantacion de una protesis total no cementada es un proceso de origen multifactorial que depende:   &#8211; de la tecnica quirurgica y tipo de implante.   &#8211; Del periodo de descarga.   &#8211; Del stock oseo inicial, en funcion del sexo y la presencia de enfermedades del metabolismo oseo.  se realiza un estudio prospectivo, controlado y de tipo mixto para determinar la influencia del dise\u00f1o del implante y el tipo de recubrimiento en los cambios de la masa osea periprotesica a lo largo de los 12 meses tras la intervencion. Igualmente, se determina el papel de la masa osea inicial como factor predictivo de la masa osea final en funcion del sexo y la edad del paciente.  los resultados demuestran una conservacion superior de masa osea en femur proximal para vastagos de apoyo metafisario, frente a los de apoyo diafisario. Por otro lado, con vastagos de apoyo proximal, se observa que en varones, con femures mas rigidos y con modulo oseo mas elevado, se consigue una fijacion del implante a nivel metafisario, con transmision de fuerzas y conservacion osea a ese nivel. En tanto que en mujeres, con femures mas peque\u00f1os y menos rigidos, la metafisis femoral no proporciona un anclaje solido del vastago, que se ajusta en la zona de transicion metafiso-diafisaria, donde transmite las fuerzas, ocasionando puenteo de fuerzas en el femur proximal y atrofia osea por desuso.  como hallazgo, rese\u00f1ar que el estudio densitometrico preoperatorio demostro diferencias significativas de masa osea entre la cadera afecta y la contralateral si era sana. Estas diferencias, atrofia del trocanter mayor e hipertrofia del calcar por sobrecarga, se interpretaron como cambios estructurales del femur para adaptarse a la nueva situacion biomecanica que
