La disponibilidad de calorimetria indirecta, determinando el consumo de oxigeno mediante el analisis de los gases respirados, de forma independiente de la determinacion del transporte de oxigeno, excluye la posibilidad de acoplamiento matematico y la termodilucion, con termistor de respuesta rapida, nos permite la mejor caracterizacion de la funcion cardiaca derecha, conociendo fraccion de eyeccion y volumenes. Con esta metodolog\u00eda, esta tesis doctoral ha pretendido clarificar las alteraciones de la hemodinamica y de la funcion cardiaca en los enfermos con shock septico mediante el analisis, en el tiempo, de la reserva funcional cardiovascular con un test de respuesta aguda a la administracion de fluidos y dobutamina, evaluando su posible implicacion pronostica, ademas de evaluar la medida del gradiente arteriovenoso de dioxido de carbono en estos pacientes y la determinacion de la presencia o no de dependencia patologica, como indicadores del grado de oxigenacion tisular periferica. El analisis de los resultados evidencio que: 1) no existe dependencia patologica del aporte de oxigeno en pacientes criticos cuando el consumo de oxigeno es determinado independientemente del aporte. 2) se ha confirmado que la disfuncion sistolica ventricular derecha es comun en el shock septico pero un deterioro de la distensibilidad caracteriza la evolucion de los pacientes que no sobreviven al episodio de shock septico. Esta disfuncion cardiaca derecha limita la reposicion de fluidos. 3) el analisis de presion arterial sistemica en respuesta a la administracion de dobutamina, a las 48 horas de evolucion, si no desciende, permite predecir la supervivencia con una probabilidad del 91%. 4) por fin, la elevacion del gradiente arteriovenoso de dioxido de carbono en el shock septico se asocia a una elevada
