Se han propuesto diversos mecanismos de neuroplasticidad, tales como la potenciaci\u00f3n de circuitos neuronales ya existentes y la inducci\u00f3n de nuevas conexiones sin\u00e1pticas.  objetivos: buscar mediante doppler transcraneal (dtc) signos de una posible activaci\u00f3n compensadora de regiones cerebrales no da\u00f1adas en diferentes etapas de la enfermedad cerebrovascular.  material y metodos: se estudiaron 30 pacientes (19 varones y 11 mujeres; edad: 64,9 +- 11,9 a\u00f1os) con hemiparesia por un ictus en la primera semana, al mes y a los seis meses de evoluci\u00f3n. As\u00edmismo se estudiaron 30 controles sanos con distribuci\u00f3n comparable de edades y sexos. Mediante dtc por v\u00eda transtemporal, se evaluaron los cambios relativos de velocidad media (v m) en ambas arterias cerebrales medias durante un ejercicio de oposici\u00f3n secuencial de los dedos de la mano.  resultados: 1) en los individuos sanos el ejercicio contralateral desencaden\u00f3 un incremento de v m mayor que el ipsilateral. 2) en los pacientes, a lo largo de toda la evoluci\u00f3n, el incremento que provoc\u00f3 el movimiento de la mano afecta (la ipsilateral) en el lado sano fu\u00e9 notorio y similar al que provoc\u00f3 el de la mano normal (la contralateral). 3) en el hemisferio lesionado, el incremento de vm fu\u00e9 significativamente mayor con la actividad de la mano afecta (la contralateral) que con la de la mano normal (la ipsilateral) en la fase cr\u00f3nica, pero no en la fase aguda.  principales conclusiones: nuestros resultados apoyan la contribuci\u00f3n del hemisferio cerebral ipsilateral en la recuperaci\u00f3n motora de la mano despu\u00e9s de un ictus, ya desde fases tempranas. Adem\u00e1s, son compatibles con una participaci\u00f3n de zonas no lesionadas del hemisferio contralateral, si bien esta participaci\u00f3n se hace m\u00e1s evidente
