La importancia del estudio se fundamenta en la preocupaci\u00f3n pol\u00edtica y social generada por los elevados tiempos de espera en la atenci\u00f3n p\u00fablica sanitaria. Para ello, es necesario en primer lugar analizar peculiaridades organizativas de los centros hospitalarios, centr\u00e1ndose fundamentalmente en su car\u00e1cter burocr\u00e1tico, la gesti\u00f3n de personal y en el sistema de incentivos empleados. Partiendo de este marco te\u00f3rico y ampliando la informaci\u00f3n sobre este tipo de sistemas mediante entrevistas en profundidad a personal directivo y sanitario que presta sus servicios en el sistema nacional de salud espa\u00f1ol, se elabora un modelo de simulaci\u00f3n empleando la metodolog\u00eda aportada por la din\u00e1mica de sistemas.  a partir de este modelo de simulaci\u00f3n y, tras someterlo a un test de comportamiento estad\u00edstico, se analiza la sensibilidad del sistema a los principales par\u00e1metros, proceso que permite detectar los puntos de apalancamiento del mismo, sobre los cuales es posible actuar para mejorar su evoluci\u00f3n. Asimismo, se analizan los efectos de las pol\u00edticas actualmente empleadas para la mejora en la evoluci\u00f3n de las listas de espera y se plantean pol\u00edticas alternativas que logran eliminar la tendencia creciente observada en la misma. Los resultados obtenidos de la simulaci\u00f3n con el modelo avalan la hip\u00f3tesis central que aboga por la necesidad de un cambio estructural en el sistema y critica el empleo de medidas puntuales
