La asociaci\u00f3n de marcadores bioqu\u00edmicos que present\u00f3 mejores criterios de discriminaci\u00f3n para el screening de madres portadoras de fetos afectados con s\u00edndrome de down fue la combinaci\u00f3n de afp y hcg total a un cutt-off de 1\/190, con una sensibilidad de 66.66% y una tasa de falsos positivos de 2.27%. El uso de la fracci\u00f3n libre de la hcg en lugar de la total a un cutt-off 1\/270 mejora la tasa de detecci\u00f3n (83.33%), pero eleva el n\u00famero de falsos positivos a 10.40%. El an\u00e1lisis discriminante del cociente de los m\u00faltiplos de la mediana de hcg y afp se revel\u00f3 como un gran indicador de riesgo de s\u00edndrome de down (83,3% de sensibilidad) superando ligeramente el l\u00edmite aceptable de 5% en la tasa de falsos positivos (5,8%). El estudio de 210 muestras de l\u00edquido amni\u00f3tico de mujeres portadoras de fetos afectados de s\u00edndrome de down refleja aumentos espectaculares de las cifras de psa. Son necesarios estudios con un mayor n\u00famero de casos para una valoraci\u00f3n eficaz de estas elevaciones. El an\u00e1lisis bioqu\u00edmico de l\u00edquido amni\u00f3tico en mujeres con riesgo de ser portadora de defectos del tubo neural puede confirmar esta patolog\u00eda por los aumentos de la fosfatasa
