{"id":54036,"date":"2018-03-09T22:41:45","date_gmt":"2018-03-09T22:41:45","guid":{"rendered":"https:\/\/www.deberes.net\/tesis\/sin-categoria\/estrategias-terapeuticas-en-carcinoma-de-mama-comportamiento-fenotipico-y-beneficio-del-trastuzumab\/"},"modified":"2018-03-09T22:41:45","modified_gmt":"2018-03-09T22:41:45","slug":"estrategias-terapeuticas-en-carcinoma-de-mama-comportamiento-fenotipico-y-beneficio-del-trastuzumab","status":"publish","type":"post","link":"https:\/\/www.deberes.net\/tesis\/histopatologia\/estrategias-terapeuticas-en-carcinoma-de-mama-comportamiento-fenotipico-y-beneficio-del-trastuzumab\/","title":{"rendered":"Estrategias terapeuticas en carcinoma de mama: comportamiento fenotipico y beneficio del trastuzumab"},"content":{"rendered":"<h2>Tesis doctoral de <strong>  Hernandez S\u00e1nchez Sonia M. <\/strong><\/h2>\n<p>Nuestro objetivo era analizar en 165 pacientes con c\u00e1ncer de mama, los m\u00e9todos de captaci\u00f3n diagn\u00f3stica a diferentes edades; estudiar el fenotipo tumoral (ihq; re\/rp, p53, ki-67, her-2 (cb-11 y herceptsettm) y top2); y su relaci\u00f3n, con caracter\u00edsticas de la paciente, del tumor, tratamiento aplicado (cirug\u00eda, quimioterapia (neoadyuvante o adyuvante), radioterapia y tratamiento hormonal), y evoluci\u00f3n de la enfermedad; adem\u00e1s, del beneficio del trastuzumab en 20 mujeres con c\u00e1ncer de mama metast\u00e1sico. el c\u00e1ncer de mama fue detectado por la propia paciente en el 69,1% de casos, correspondiendo al m\u00e9todo mamogr\u00e1fico el 20,6% (11,5%-programa de cribado mamogr\u00e1fico de la junta de cstilla y le\u00f3n). Por ello, la exploraci\u00f3n mamaria peri\u00f3dica por profesionales sanitarios, es determinante, a edades fuera de scrrening y el en intervalo entre mamograf\u00edas. la media de edad fue de 59 a\u00f1os (40,6% entre 45-65), con tumores espor\u00e1dicos (91%), en enfermedad locoregional (81%), tama\u00f1o tumoral medio de 3cm, cdi (76%, grado iii-40%), ganglios linf\u00e1cticos positivos (38%), fenotipo hormonal re+\/rp+ (60%), p53 alterado (44%), ki-67 moderado-alto (57%), expresi\u00f3n de top2 (26,7%), de her-2 (cb-11: 43,6%: herceptesttm: 32,1%, concordancia entre t\u00e9cnica del 77,6% que aumenta al comprobar los casos dudosos herceptesttm 2+ con m\u00e9todos de hibridaci\u00f3n in situ cish y fish). Conforme se reduce la expresi\u00f3n de los receptores hormonales, aumenta tama\u00f1o, grado histol\u00f3gico, proliferaci\u00f3n celular y expresi\u00f3n de her-2 y top2. Las mujeres j\u00f3venes, tienen ki-67 moderado-alto, her-2 y top2 positivos. la mayor\u00eda reciben cirug\u00eda radical (53%), qt (adyuvante (69%) [cmf (40%), scaf\/caf (34%)], rt (64%) y tratamiento hormonal (71%; tamoxifeno (66%)). Existe modificaci\u00f3n de niveles de p53 y top2 con la administraci\u00f3n de agentes quimioter\u00e1picos. El fenotipo de fallecidas: p53 alterado, her-2 y top2 negativos.
