{"id":56484,"date":"2007-01-01T00:00:00","date_gmt":"2007-01-01T00:00:00","guid":{"rendered":""},"modified":"2007-01-01T00:00:00","modified_gmt":"2007-01-01T00:00:00","slug":"evaluacion-de-genotype-mycobacterium-direct-nueva-tecnica-genetica-de-microbiologa%c2%ada-molecular-en-la-deteccion-de-m-tuberculosis-complex-y-otras-micobacterias-ata%c2%adpicas-en-muestras-de-inte","status":"publish","type":"post","link":"https:\/\/www.deberes.net\/tesis\/microbiologia-clinica\/evaluacion-de-genotype-mycobacterium-direct-nueva-tecnica-genetica-de-microbiologa%c2%ada-molecular-en-la-deteccion-de-m-tuberculosis-complex-y-otras-micobacterias-ata%c2%adpicas-en-muestras-de-inte\/","title":{"rendered":"Evaluaci\u00f3n de genotype mycobacterium direct, nueva t\u00e9cnica gen\u00e9tica de microbiolog\u00eda molecular en la detecci\u00f3n de m. tuberculosis complex y otras micobacterias at\u00edpicas en muestras de inter\u00e9s cl\u00ednico"},"content":{"rendered":"<h2>Tesis doctoral de <strong> Francisco Franco Alvarez De Luna <\/strong><\/h2>\n<p>Nuestro objetivo es evaluar una nueva t\u00e9cnica gen\u00e9tica, genotype mycobacterium direct (gtmd), basado en la tecnolog\u00eda nasba (nucleic acid sequence-based amplification) que nos permite mediante la amplificaci\u00f3n de 23s rarn, la detecci\u00f3n de m.Tuberculosis complex, m.Avium, m,intracellulare, m.Kansasii, y m.Malamoense directamente de los espec\u00edmenes cl\u00ednicos se analizaron 186 muestras respiratorias y extrapulmonares, procedentes de 89 pacientes sospechosos de padecer tuberculosis u otro tipo de micobacteriosis.     esta t\u00e9cnica se divide en tres partes:     1,- estabilizaci\u00f3n y el aislamiento de arnr.     2,- amplificaci\u00f3n isot\u00e9rmica del arn mediante nasba, a 41\u00c2\u00bac.     3,- detecci\u00f3n del producto amplificado mediante hibridaci\u00f3n reversa sobre membrana de nitrocelulosa.     de las 186 muestras totales, 138 dieron cultivo positivo para micobacterias. En 130 muestras se obtuvo crecimiento para m.Tuberculosis complex. Las muestras restantes fueron positivas para m.Intracellulare (3), m.Malmoense (1), m.Kansasii (1) y m.Avium (3) gtmd detect\u00f3 un total de 135 muestras positivas. De \u00e9stas, 14 muestras fueron positivas donde el cultivo  de micobacterias fue negativo. Esto indica una sensibilidad, especificidad, vpp y vpn de gtmd respecto al cultivo del 88%, 71%, 91% y 74% respectivamente. para el an\u00e1lisis de las discrepancias encontradas (resultados gtmd positivos como cultivo negativo) se consider\u00f3 un verdadero positivo para la t\u00e9cnica de amplificaci\u00f3n cuando los pacientes era sospechosos de padecer tuberculosis pulmonar activa tras una revisi\u00f3n de la historia cl\u00ednica, respondieron al tratamiento antituberculoso o el paciente tuvo alg\u00fan, cultivo positivo en los \u00faltimos 6 meses anteriores al estudio. Las 14 muestras fueron consideradas como verdaderos positivos, resultando la sensibilidad, especificidad, vpp y vpn al 88%, 100% y 63% respectivamente. gtmd es una t\u00e9cnica r\u00e1pida (4-5h), ya
