El objetivo de este trabajo fue dar a conocer la situaci\u00f3n epidemiol\u00f3gica de la enfermedad meningoc\u00f3cica en gipuzkoa, comunidad aut\u00f3noma del pa\u00eds vasco, entre los a\u00f1os 1988 y 2003, estudiar fenot\u00edpica y genot\u00edpicamente las cepas causantes de enfermedad y las aisladas en la faringe de portadores asintom\u00e1ticos. la enfermedad meningoc\u00f3cica en gipuzkoa mostr\u00f3 el patr\u00f3n end\u00e9mico t\u00edpico de las zonas desarrolladas. Incidencia baja, con una media anual inferior a 5 casos confirmados x 100.000 habitantes. La mortalidad global fue del 6,3% y la de los ni\u00f1os del 2,2%. Durante el \u00faltimo trienio, coincidiendo con la vacunaci\u00f3n con vacuna conjugada frente al meningococo c en el calendario vacunal y con a circulaci\u00f3n de un nuevo clon hipervirulento en gipuzkoa, este patr\u00f3n end\u00e9mico t\u00edpico se vio alterado. La incidencia global permaneci\u00f3 baja, pero disminuyeron los episodios infantiles y aumentaron los de adultos. La forma de presentaci\u00f3n mayoritaria fue la s\u00e9ptica sin meningitis, aumentando la mortalidad. Se analizaron 398 episodios confirmados microbiol\u00f3gicamente. Los serogrupos b y c fueron mayoritarios entre las cepas invasivas. El an\u00e1lisis gen\u00e9tico mostr\u00f3 escasa diversidad entre ellas, ya que m\u00e1s de la mitad de los episodios estuvieron causados por ocho clones pertenecientes a complejos clonales con reconocida capacidad virulenta. La poblaci\u00f3n de meningococos aislados en la faringe de portadores asintom\u00e1ticos mostr\u00f3 mayor diversidad fenot\u00edpica que las cepas causantes de
