La tecnica doppler permite analizar el llenado ventricular izquierdo y derecho.  objetivo: analisis doppler del flujo de la vena cava superior (fvcs), en la deteccion de rechazo agudo en el trasplante cardiaco. Metodos: estudio prospectivo de 70 pacientes (p) trasplantados valorando el ventriculo izquierdo (vi) y ambas auriculas, con ecocardiografia.  tambien se valoro el llenado mitral, tricuspide y del (fvcs). Se tomaron 145 biopsias cardiacas en 51 p.  resultados: se registro un 9% de rechazo &#8211; moderado. El tama\u00f1o del ventriculo izquierdo fue normal. La fraccion de eyeccion fue &#8211; 50% en 96% de los estudios. La fraccion de cambio de area de ambas auriculas fue &lt; 20% en la mayoria. La velocidad pico y la integral de la onda de llenado inicial de la curva de llenado mitral mostraban un valor predictivo negativo del 96% para la deteccion de rechazo moderado. El fvcs mostro predominio de la onda diastolica de flujo anterogrado; el 50% mostro una onda de contraccion auricular, y el 40% mostro una onda retrograda sistolica. Los parametros del fvcs fueron similares con y sin rechazo, en apnea y en inspiracion.  en 17 p (24%), el fvcs mostro una gran variabilidad interlatido. Conclusion: la evaluacion doppler del fvcs, no es util en la deteccion de rechazo agudo.
