Este estudio analiza el valor de la pet-18fdg en el diagn\u00f3stico de la afectaci\u00f3n ganglionar axilar en 275 pacientes diagnosticadas de c\u00e1ncer de mama. Las im\u00e1genes de la pet se obtienen tras la administraci\u00f3n de 370 mbq de 18fdg; se eval\u00faa el \u00e1rea mamaria, axilar y las cadenas ganglionares, mamaria interna y supraclavicular. En un subgrupo de 125 pacientes el estudio mediante la pet se complementa con la realizaci\u00f3n de una biopsia del ganglio linf\u00e1tico centinela (bglc) en aquellos casos en que no se demostr\u00f3 captaci\u00f3n patol\u00f3gica a nivel axilar. La sensibilidad y especificidad de la pet-fdg en el diagn\u00f3stico de la afectaci\u00f3n axilar fue de 84,5% y 98,5% respectivamente con 22 casos falsos negativos. La probabilidad de obtener un resultado falso negativo en el estudio de la axila mediante la pet es significativamente mayor en los tumores peque\u00f1os (ti), con suv menor o igual a 3,5, en los de tipo lobulillar y en aquellos con expresi\u00f3n de receptores de estr\u00f3genos. En 21 pacientes la pet-fdg detect\u00f3 captaci\u00f3n patol\u00f3gica a nivel de los ganglios de la cadena mamaria interna. Se encontr\u00f3 relaci\u00f3n significativa entre el suv (valor de captaci\u00f3n estandarizado) de la pet y otros factores de inter\u00e9s pron\u00f3stico en el c\u00e1ncer de mama como el tama\u00f1o tumoral, grado y tipo histol\u00f3gico, expresi\u00f3n de receptores de estr\u00f3genos y proliferaci\u00f3n celular medida mediante la expresi\u00f3n de ki67. Dada su alta especificidad la pet-fdg evit\u00f3 la bglc en aquellos pacientes con captaci\u00f3n patol\u00f3gica axilar a los que se le realizo directamente vaciamiento axilar est\u00e1ndar. En los casos de pet negativo a nivel axilar hay que complementar el estudio con la bglc. La asociaci\u00f3n de ambas t\u00e9cnicas mejora mucho las tasas de sensibilidad y especificidad en el estudio de la afectaci\u00f3n axilar y la utilidad de la pet puede extenderse tambi\u00e9n a la
