El neuroma postamputacion ocasiona dolor invalidante en el 10-20% de los casos, cuya solucion adecuada sigue planteando un reto terapeutico. La anastomosis centro-central con autoinjerto fascicular (acc) se ha mostrado util en el animal de experimentacion, asi como en escasas experiencias en humanos sobre nervios digitales. Material y metodo. Se estudian 47 pacientes afectos de neuroma de amputacion a los que se les habia practicado una acc. La tecnica consiste en la diseccion del neuroma, practicando despues la diseccion de un numero par de fasciculos uniendo ambos cabos de cada par, practicando luego una seccion completa de un fasciculo de cada par a 1-2 cm de forma que queda un autoinjerto intermedio. Posteriormente se suturan los cabos. En los nervios digitales, la acc se practico entre los dos colaterales. Resultados. A un seguimiento medio de 9,4 meses, 38 pacientes se consideraron curados, 3 mejorados y 6 con fracaso.
