El primer paso para que un carcinoma invasivo establezca una met\u00e1stasis es que sea capaz de degradar la membrana basal subyacente, compuesta principalmente de col\u00e1geno tipo iv. Las metaloproteasas 2 y 9 (mmp-2, -9) juegan un importante papel en la degradaci\u00f3n de dicha membrana, ya que son capaces de romper este tipo de col\u00e1geno. Los inhibidores titulares de metaloproteasas (timps) tienen la capacidad de inhibir a las mmps, pero parad\u00f3jicamente, tambi\u00e9n pueden ser indicadores de un mal pron\u00f3stico.Material y m\u00e9todo: se ha estudiado la expresi\u00f3n de las mmp-2 y -9 y la de los timp-1 y timp-2 por medio de la inmunohistoqu\u00edmica en una serie de 68 casos de carcinomas de c\u00e9lulas escamosas de la cavidad oral con la intenci\u00f3n de evaluar su significado pron\u00f3stico.Resultados: el 28% de los carcinomas orales (n= 19) expresaron mmp-2, y el 17,6% (n= 12) expresaron mmp-9. La inmunorreacci\u00f3n de la mmp-2 fue significativamente m\u00e1s elevada en los pacientes que eran consumidores de alcohol (p= 0,028) (or= 4) y en aquellos menores de 60 a\u00f1os (p= 0,041). la inmunorreacci\u00f3n de la mmp-9 mostr\u00f3 asociaci\u00f3n significativa con el grado de diferenciaci\u00f3n histol\u00f3gica tumoral (p= 0,019), el tama\u00f1o tumoral (grado t) (p= 0,05) y tambi\u00e9n con el consumo de alcohol (p= 0,04) (or= 7,67). tambi\u00e9n se observ\u00f3, aunque no de forma estad\u00edsticamente significativa, que la inmunoexpresi\u00f3n de mmp-2 y mmp-9 era m\u00e1s elevada en los pacientes con met\u00e1stasis linf\u00e1ticas (or= 1,65 y 2,29 respectivamente). En los pacientes sin met\u00e1stasis linf\u00e1ticas, la inmunorreacci\u00f3n de la mmp-9 se relacion\u00f3 con peores tasas de supervivencia (p= 0,02; or= 5,8).
