{"id":80064,"date":"2018-03-09T23:25:58","date_gmt":"2018-03-09T23:25:58","guid":{"rendered":"https:\/\/www.deberes.net\/tesis\/sin-categoria\/resistencia-a-la-insulina-en-la-hipertension-refractaria-efecto-del-tratamiento-con-pioglitazona\/"},"modified":"2018-03-09T23:25:58","modified_gmt":"2018-03-09T23:25:58","slug":"resistencia-a-la-insulina-en-la-hipertension-refractaria-efecto-del-tratamiento-con-pioglitazona","status":"publish","type":"post","link":"https:\/\/www.deberes.net\/tesis\/sin-categoria\/resistencia-a-la-insulina-en-la-hipertension-refractaria-efecto-del-tratamiento-con-pioglitazona\/","title":{"rendered":"Resistencia a la insulina en la hipertensi\u00f3n refractaria. efecto del tratamiento con pioglitazona"},"content":{"rendered":"<h2>Tesis doctoral de <strong>  Rivas Otero Beatriz De <\/strong><\/h2>\n<p>Prop\u00f3sito del estudio     el objetivo del estudio fue valorar el efecto de la pioglitazona sobre la presi\u00f3n arterial (pa), la resistencia a la insulina y otros factores de riesgo cardiovascular en pacientes con hta refractaria y diabetes mellitus tipo 2 (dm).  m\u00e9todos usados     en 27 pacientes con dm y hta refractaria (mapa de 24 horas menos 125\/75 mmhg) en tratamiento con antihipertensivos (media: 4,1 +- 0,8 f\u00e1rmacos), se (homar-ir), y an\u00e1lisis sangu\u00edneos de glucosa, insulina, hba1c, l\u00edpidos y par\u00e1metros habituales de laboratorio.  resultados     tras 20 semanas con pioglitazona, se observ\u00f3 un descenso significativo de la pa en la mapa de 24 horas (pas: de 144,35 +- 13,14 a 135,92 +- 15,87 mmhg, p=0,001; pad: de 79,46 +- 8,89 vs 7565 +- 10,45 mmhg, p=0,001). Estas reducciones fueron mayores en los pacientes con resistencia inicial a la insulina. la resistencia a la insulina (homar-ir) se redujo de forma significativa (de 4,47 +- 2,78 a 2,85 +- 1,37; p=0,001). Tambi\u00e9n se redujeron la glucosa en ayunas (de 151,5 +- 38,85 a 139,48 +- 30,84 mg\/dl: p=0,042), la insulina (de 11,1 +- 5,54 uu\/ml (p=0,001) y la hba1c (de 6,56 +- 0,89 a 6,29 +- 0,77; p=0,057). las cifras de triglic\u00e9ridos descendieron (de 208,3 +- 304,9 a 135,12 +- 66,67 mg\/dl; p=0,016) y aumentaron las de hdl (de 47,7 +- 10,41 a 56 +- 10,57 mg\/dl; p menor 0,001). No hubo cambios en el colesterol total, ldl y la apolipoprote\u00edna b.  conclusiones     estos resultados sugieren que la resistencia a la insulina juega un papel importante en la hipertensi\u00f3n refractaria de los pacientes diab\u00e9ticos.
