A pesar de la gran cantidad de estudios realizados, los mecanismos responsables del desencadenamiento y progreso de la hepatopat\u00eda alcoh\u00f3lica no est\u00e1n a\u00fan del todo caracterizados. La oxidaci\u00f3n del etanol en el hepatocito da lugar un estr\u00e9s oxidativo y una lipoperoxidaci\u00f3n celular. paralelamente, y como resultado del metabolismo del etanol, se producen importantes alteraciones en la mitocondria que pueden contribuir a incrementar este estr\u00e9s oxidativo, llegando a comprometer la viabilidad celular. Puesto que el glutation mitocondrial (gshm) es la \u00fanica defensa frente a esta producci\u00f3n de radicales libres (ros), la limitaci\u00f3n de los niveles de gshm podr\u00eda tener consecuencias adversas en el mantenimiento de la viabilidad celular.  estudios previos realizados en modelos animales de intoxicaci\u00f3n cr\u00f3nica de etanol, mostraban una disminuci\u00f3n selectiva del gshm, sin que se produjera ninguna alteraci\u00f3n en los niveles citos\u00f3licos. Esta disminuci\u00f3n preced\u00eda tanto la disfuncionalidad mitocondrial como la lesi\u00f3n hep\u00e1tica. Al analizar el gshm en situaciones de producci\u00f3n de estr\u00e9s oxidativo, observamos que jugaba un papel fundamental en el mantenimiento de la funcionalidad mitocondrial, al controlar la disponibilidad de ros, en concreto per\u00f3xido de hidr\u00f3geno, generados desde el complejo iii de la cadena respiratoria. Asimismo, la limitaci\u00f3n del gshm, favorec\u00eda la activaci\u00f3n de factores de transcripci\u00f3n como el nf-kb, por tanto, a trav\u00e9s del control del entorno oxidativo celular, el gshm interven\u00eda en la regulaci\u00f3n de genes nucleares.  estudios previos mostraban que la disminuci\u00f3n del glutation mitocondrial inducida por el alcohol era resultado de un defecto en el transporte desde el citosol a la matriz mitocondrial.
