Sin embargo, la gentamicina es nefrot\u00f3xica, siendo su principal efecto la necrosis de c\u00e9lulas del t\u00fabulo proximal. La gentamicina promueve una reducci\u00f3n del flujo plasm\u00e1tico renal (rbf) y de la tasa de filtraci\u00f3n glomerular(gfr), sin generar un da\u00f1o aparente en el glom\u00e9rulo.  el descenso en el gfr se ha atribuido, al menos en parte, a un descenso en el coeficiente de ultrafiltraci\u00f3n (kf), y su regulaci\u00f3n depende principalmente de la actividad de las c\u00e9lulas mesangiales intraglomerulares, las cuales pueden contraerse o relajarse, disminuyendo o aumentando el \u00e1rea efectiva de filtraci\u00f3n(s). Estudios previos de nuestro laboratorio y otros mostraron que la gentamicina estimula la liberaci\u00f3n de no, radicales libres de ox\u00edgeno(ros) yla s\u00edntesis de paf en c\u00e9lulas mesangiales, y tambi\u00e9n induce  contracci\u00f3n y proliferaci\u00f3n. Proponemos que este aminoglic\u00f3sido puede tambien inducir apoptosis, y que estas sustancias pueden ser mediadoras tanto de proliferaci\u00f3n como de la adptosis en c\u00e9lulas mesangiales.  la gentamicina induce contracci\u00f3n y proliferaci\u00f3n de c\u00e9lulas mesangiales de rata en cultivo. Ambos efectos estan mediados por la activaci\u00f3n de la fosfolipasa a2, y un incremento simult\u00e1neo en la s\u00edntesis y liberaci\u00f3n de paf. La gentamicina promueve un incremento en la concentraci\u00f3n de calcio libre citos\u00f3lico, el cual es un mediador de la respuesta contr\u00e1ctil y proliferativa de las c\u00e9lulas mesangiales, y posiblemente, de la activaci\u00f3n de la fosfolipasa a2; la fuente de este calcio es tanto el medio extracelular como los reservorios intracelulares. La gentamicina estimula tambi\u00e9n apaptosis en c\u00e9lulas mesangiales en cultivo; esta apaptosis puede actuar como un mecanismo compensatorio de la proliferaci\u00f3n mesangial, y los ros son uno de los mediadores de este proceso.
