Jes\u00fas Moro Alvarez <\/strong><\/h2>\n<p>Estudios previos apuntan a que el tratamiento cr\u00f3nico con hidantoinas podr\u00eda ser un factor de riesgo para desarrollar osteoporosis y\/o osteomalacia. ultimamente se est\u00e1n desarrollando nuevas t\u00e9cnicas, que aportan importantes innovaciones en el diagn\u00f3stico de la osteoporosis (densitometr\u00eda, resistencia a la torsi\u00f3n, marcadores biol\u00f3gicos de remodelado \u00f3seo, etc.) Estas t\u00e9cnicas no hab\u00edan sido hasta el momento aplicadas para el estudio del efecto de las hidantoinas sobre el hueso. Con objeto de investigar la patofisiolog\u00eda de la osteopenia inducida por f\u00e1rmacos anticonvulsivos realizamos un estudio experimental y cl\u00ednico para confirmar estos hechos  con la nueva metodolog\u00eda actual.  examinamos a 30 pacientes que hab\u00edan recibido tratamiento con fenitoina durante al menos 5 a\u00f1os. Encontramos que un alto porcentaje de estos pacientes presentan una disminuci\u00f3n de la densidad mineral \u00f3sea en cuello femoral, afectando preferentemente a los varones de mayor edad. La preValencia de osteoporosis en nuestra poblaci\u00f3n fue del 3.3% y de osteopenia del 56.6%. los pacientes presentan un aumento de los marcadores de resorci\u00f3n, con un desacoplamiento en el remodelado que justifica la osteopenia observada. presentan adem\u00e1s alteraciones del metabolismo fosfoc\u00e1lcico, con tendencia a la hipocalcemia e hiperparatiroidismo secundario, con niveles de 25(oh)d y 1,25(oh)(sub2)d normales.  a nivel experimental la administraci\u00f3n de fenitoina durante 6 semanas a dosis de 5 mg\/kg\/d a ratas wistar macho en fase de crecimiento, provoca igualmente una disminuci\u00f3n de la densidad mineral \u00f3sea y del contenido mineral \u00f3seo en f\u00e9mur. En las biopsias vertebrales no provoca osteomalacia, pero produce una disminuci\u00f3n del volumen trabecular y la disminuci\u00f3n de la formaci\u00f3n \u00f3sea con tendencia a aumentar la resorci\u00f3n \u00f3sea.
