Sin embargo los mecanismos moleculares de la muerte del epitelio tubular renal son desconocidos. La comprensi\u00f3n de estos mecanismos puede llevar al dise\u00f1o de nuevas estrategias terap\u00e9uticas en pacientes con insuficiencia renal, una situaci\u00f3n cl\u00ednica con enormes implicaciones econ\u00f3micas y sociales. hemos estudiado la expresi\u00f3n de prote\u00ednas reguladoras de la muerte celular durante el fra y hemos investigado, en c\u00e9lulas tubulares  cultivadas, cu\u00e1les son los agentes responsables de los cambios observados in vivo, las consecuencias funcionales de esos cambios y posibles maniobras terap\u00e9uticas. Durante el fra y en la nefritis intersticial cr\u00f3nica se producen localmente en el ri\u00f1on citoquinas letales, como tnf y fasl. Adem\u00e1s, en el fra la expresi\u00f3n de prote\u00ednas de muerte celular de la familia bcl2 est\u00e1 alterada , aumenta la prote\u00edna proapopt\u00f3tica bax, disminuye la prote\u00edna antiapopt\u00f3tica bcl2 y se producen cambios heterog\u00e9neos en la expresi\u00f3n de bclxl: algunas c\u00e9lulas tubulares pierden la expresi\u00f3n de bclxl mientras que otras, probablemente las que luego regeneran los t\u00fabulos, tienen una expresi\u00f3n aumentada de esta prote\u00edna protectora. En c\u00e9lulas tubulares epiteliales cultivadas comprobamos que los factores de supervivencia del suero evitan la muerte celular, y su ausencia crea un microambiente intracelular, caracterizado por un bajo cociente bclxl\/bax, que favorece la muerte por apoptosis y predispone a la apoptosis inducida por citoquinas letales y nefrotoxinas tambi\u00e9n disminuyen la expresi\u00f3n de la prote\u00edna antiapopt\u00f3tica bclxl. Al explorar las posibles consecuencias terap\u00e9uticas de estos hallazgos, observamos que la sobreexpresi\u00f3n de bclxl protege al epitelo tubular de la apoptosis inducida por citoquinas letales y nefrotoxinas.
