En el estudio caso control que realizamos los factores significativamente asociados con la refractariedad fueron la presencia de anticuerpos anti-hla, el trasplante de medula \u00f3sea por leucemia mieloide cr\u00f3nica y la presencia de fiebre. El an\u00e1lisis de la clase de anticuerpos mostr\u00f3 igg en 9\/18 pacientes estudiados, igm en 6\/18 e igg+igm en 3\/18. la subclase de igg fue igg1 en 10, igg1+igg1+igg4 en otro. No hubo diferencias significativas en el rendimiento de la transfusi\u00f3n de plaquetas en funci\u00f3n de la clase de anticuerpo. Tampoco se encontr\u00f3 correlaci\u00f3n entre el rendimientode la transfusi\u00f3n y la concentraci\u00f3n de anticuerpos. Los mecanismos de acci\u00f3n de los anticuerpos anti-hla son hetereog\u00e9neos. En algunos pacientes son capaces de producir trombocitolisis y de activar las plaquetas (6 de los 17 pacientes estudiados), efectos que probablemente est\u00e9n mediados por la activaci\u00f3n del complemento. Sin embargo, en la mayor\u00eda de los pacientes alinmunizados la destrucci\u00f3n de las plaquetas transfundidas est\u00e1 mediada por otros mecanismos.
