La finalidad del estudio fue valorar la reserva\/transfusion y la reserva\/transfusion adecuada de concentrados de hematies en algunos procedimientos quirurgicos con la finalidad de: 1\u00c2\u00ba determinar el test de reserva y 3\u00c2\u00ba, analizar, a partir de datos demograficos, clinicos y analiticos, algunos de los factores predictores de trasfusion perioperatoria, para asi poder realizar una estrategia personalizada desde el banco de sangre. Para ello, realizamos el presente trabajo en el hospital marina alta, un hospital comarcal de 120 camas de hospitalizacion, perteneciente al servicio Valenciano de salud. Se trata de un estudio observacional, retrospectivo y prospectivo y analitico de la transfusion perioperatoria de concentrados de hematies en cirugia programada. Se estudio a 1034 pacientes entre enero 1995 y octubre de 1998 sometidos a diferentes procedimientos quirurgicos con anestesia general y reserva preoperatoria de sangre, previamente se emiti\u00f3 desde el comit\u00e9 hospitalario de transfusion un protocolo de indicaciones de transfusion perioperatoria de concentrados de  hematies. Las conclusiones de nuestro estudio son las siguientes. 1-se detecta un exceso de reserva de unidades y un exceso de test de reserva en cirugia programada, el test de reserva global de eleccion es el tipaje y escrutinio de anticuerpos irregulares. 2-el nivel de hemoglobina postoperatoria mas bajo se detect\u00f3 en los pacientes en los que se realiz\u00f3 resecci\u00f3n abdominoperineal. 3-se realiz\u00f3 transfusion periopertoria en: mastectomias (1,2%), histerectomias (10,4%), cirugia urol\u00f3gica (11%), cirugia ortopedica y traumatologica (37,7%), cirugia colorrectal (46,7%). La incidencia de transfusion no se modifico en los a\u00f1os de estudio, pero si aumento el porcentaje de transfusiones adecuadas.
