Los segundos tumores primarios (stp) y las recidivas tumorales (rt) tienen gran trascendencia en la progresi\u00f3n de la enfermedad y en la supervivencia de los pacientes. objetivos: estudiar los cambios g\u00e9nicos en stp y rt para contribuir al conocimiento de la progresi\u00f3n tumoral, con posibles implicaciones en el diagn\u00f3stico precoz y en el pron\u00f3stico. metodolog\u00eda: se estudian 36 pacientes con cecc, 21 desarrollaron stp y 15 rt. Se excluyeron los que hab\u00edan recibido radioterapia entre ambos diagn\u00f3sticos. Empleamos la t\u00e9cnica de mlpa que utiliza una m\u00ednima cantidad de adn (>20 ng) y permite amplificar m\u00e1s de 40 secuencias distintas de adn, obteniendo un patr\u00f3n de p\u00e9rdidas\/ganancias. resultados: los stp tienen mayor supervivencia (67%) que las rt (20%). los cambios g\u00e9nicos que diferencian los cecc iniciales de los stp fueron las p\u00e9rdidas en il2, ai651963, il18, brca2, dleu, cdh2 y la ganancia de cdkn2d. Para las rt las p\u00e9rdidas en igsf4 y tp53 y las ganancias en lmna y ptpn1. los cambios g\u00e9nicos que diferencian los stp y las rt fueron las p\u00e9rdidas en tp53 (ex\u00f3n 11), cdkn2a, ai651963, rent2, il18 y las ganancias en lmna, ctsb, ptpn1 y n33. entre los cecc con su correspondiente stp y rt predominan las discordancias, aunque tambi\u00e9n se observan concordancias. conclusiones: el estudio realizado demuestra buen n\u00famero de cambios g\u00e9nicos que diferencian los grupos estudiados, algunos no descritos previamente. El gran n\u00famero de discordancias entre los cecc y sus rt, junto con algunas concordancias entre cecc y stp, sugieren que los criterios cl\u00ednicos de progresi\u00f3n tumoral no son adecuados y
